Academic Progress Report (APR)

Dear New & Continuing EOPS Students,
Starting on Monday, March 20th, the Academic Progress Report (APR) requirement will begin. You will have until Wednesday, April 26th at 3:00pm (and we do mean 3pm) to turn in a completed progress report to the EOPS office (see form attached). This is a very important tool that will give you an indication of your academic status in your courses. It will also help your counselors and your instructors monitor your academic progress, coordinate interventions, and make appropriate referrals (i.e.; tutoring) if necessary.

INSTRUCTIONS:

 1)     Print the Form by CLICKING HERE  (or pick one up in the EOPS office ASAP).

2)     Fill in your information at the top of the form and then list your classes below.

3)     Take it to your instructor(s) or to the course assistant before class, after class or during office hours ONLY.  Ask them to record your progress. Please remember to have them sign it.

a)     If your instructor does not have a grade recorded for you yet, you MUST have the instructor write "No grade recorded at this time" and sign it.

b)     If you are taking classes online, please print your current grade report from Blackboard, if no grades have been assigned, please have your instructor email you confirmation of this, then print the email and attach it to the APR.  

c)     In addition, if you have a class you attend in person, and that teacher has grades available on Blackboard, you may also print this out and attach it to your APR.

NOTE: Please be respectful of your instructor and fellow students.  Also, plan ahead! You are being notified early and should not be completing this EOPS requirement last minute.  One month is more than sufficient time to complete this requirement.  As always, we are here if you need clarification or have any questions about this requirement.

HOW TO GET CREDIT:

 ·       IF your progress report indicates satisfactory progress (ALL grades A, B, C, or Pass)…………….

Turn the report form in to anyone at the EOPS front desk.  They will give you a receipt to keep for your records, as proof that that your APR was passing and turned in on time.  This receipt will count as one of your 3 required counseling contacts for the semester.

 ·       IF your progress report indicates Unsatisfactory progress (1 or more D, F, or No Pass)…… You must see an EOPS academic counselor before submitting the report. Remember, the EOPS counselor will assist you in improving your performance in your classes. The EOPS counselors will have walk-in hours from Monday, April 17th, 2017 through Wednesday, April 26th, 2017 for those students who need to see a counselor to discuss their APR. Once you have seen the counselor, that visit will be counted.  You MUST have your APR with you when you arrive for your walk-in visit.

PLEASE READ THESE VERY IMPORTANT TIPS!:

~ IF YOU SHOW UP ON THE DEADLINE DATE AND YOU ARE MISSING INSTRUCTOR SIGNATURES OR FORGOT YOUR APR AT HOME, YOU WILL NOT GET CREDIT AND YOU WILL BE PLACED ON PROGRAM WARNING FOR THE FOLLOWING SEMESTER. 

 ~ IF YOU SHOW UP ON THE DEADLINE DATE AND NEED TO SEE A WALK-IN COUNSELOR, BUT DO NOT HAVE AT LEAST AN HOUR TO WAIT, YOU WILL NOT GET CREDIT FOR THIS REQUIREMENT.

 ~  UNFORTUNATELY, THE MAJORITY OF STUDENTS WAIT TO TURN IN THEIR APR ON THE LAST 2 DAYS.  DO NOT DO THIS.  IF YOU DO NOT WANT A LONG WAIT TO SEE AN ACADEMIC COUNSELOR, THEN COMPLETE THIS REQUIREMENT EARLY AND TURN IT IN.  WE GIVE YOU A MONTH AND YOU CAN EASILY TURN THIS IN ON WEEK 1 OR 2. COMPLETING THIS EARLY WILL SAVE YOU TIME AND ALLOW FOR STAFF TO ASSIST ALL STUDENTS MORE EFFICIENTLY.

 ~ IF YOU ARE HAVING TROUBLE WITH OBTAININIG GRADES OR INSTRUCTOR SIGNATURE, OR YOU DON’T UNDERSTAND WHAT IS REQUIRED, DO NOT WAIT UNTIL THE DEADLINE TO ADDRESS THE ISSUES YOU ARE HAVING WITH OUR OFFICE OR YOUR SPECIALIST.

 ~ YOU DO NOT NEED TO TURN IN YOUR APR INSTRUCTION SHEET AND THE COUNSELOR WALK-IN SCHEDULE.  PLEASE ONLY TURN IN YOUR APR FORM.

STUDENTS THAT DO NOT COMPLETE APR  =  FALL 2016 PROGRAM WARNING

 The APR form must be completed and submitted to the EOPS Office by 3:00 p.m. on Wednesday, April 26th. Once you have all the grades and signatures recorded, it is recommended that you turn in the form as soon as possible so that you are not late for the deadline.  Students who do not turn it in on time will not be meeting the requirements set forth in the Mutual Responsibility Contract.  In addition, those who do not complete the APR on time, will be placed on program warning for the following semester.

 ALL EOPS STUDENTS MUST TURN IN AN APR REGARDLESS OF PASSING OR NON-PASSING GRADES
